Misfortune, by Wesley Stace. It's about a boy raised as a girl in 1820's/30's England and the ensuing hijinks when her sex becomes known. Sort of in the vein of Eugenides's Middlesex, but Rose isn't intersex the way Cal is, she just happens to be a girl who has a penis.
